**Photo Release Permission Form**
I hereby grant permission to Brentwood Baptist Church to take and use photographs and/or videos of my child, during the Vacation Bible School event held from July 22-July 26.
I understand that these photographs and/or videos may be used in various media formats, including but not limited to the church's website, social media platforms, promotional materials, and other publications.
I also understand that my child's name will not be used in conjunction with the photographs and/or videos.
I hereby release and discharge Brentwood Baptist Church, its agents, representatives, and assigns from any and all claims and demands arising out of or in connection with the use of the photographs and/or videos, including any claims for compensation.
I certify that I am the parent/legal guardian of the child named above and that I have the authority to grant the permissions stated in this release.
*Permission to Treat in Case of Injury Form**
I give permission for my child to receive emergency medical treatment in the event of an injury or illness during the Vacation Bible School event held from July 22-26, 2024
In the event that I cannot be reached in an emergency, I authorize the Vacation Bible School staff to seek medical treatment for my child, including but not limited to calling emergency services and transporting my child to a hospital or medical facility for treatment.
I understand that every effort will be made to contact me, or the emergency contact listed below in case of an injury or medical emergency involving my child.
I hereby release and hold harmless Brentwood Baptist Church, its staff, volunteers, and representatives from any and all liability for injuries or damages that may occur during the Vacation Bible School event, except those caused by willful misconduct or gross negligence.
I certify that I am the parent/legal guardian of the child named above and that I have the authority to grant the permissions stated in this form.
